Hi Bob,
I agree it isn't very clear what they want. You could put the date or the aircraft type, but you put all that info on the front page and in more detail. Usually when I see spots like this I always add more info. i.e. add both date and aircraft. It only takes a few seconds and ALL bases are covered. Remember growing up and filling out forms that they said leave no box or section blank. Same here just add the extra couple of words. It hurts nothing and complies with everything. :)
